Het postfinasteridesyndroom (PFS)[1] is een term die gebruikt wordt door patiënten, artsen en de media voor een complex van symptomen, waaronder aanhoudende seksuele, neurologische en fysieke bijwerkingen bij patiënten die finasteride, een 5-allfa-reductaseremmer voor de behandeling van haaruitval (onder de merknaam Propecia of generieke) en van een vergrote prostaat (BPH) (onder de merknaam Proscar of generieke), hebben genomen.[1][2] Deze vermoedelijk vrij zeldzame aandoening kan na stopzetting van medicatie nog maanden, zelfs jaren voortduren; maar in meeste gevallen is het permanent.[1][2][3][4][5][6][7][8]

Frequentie

Over de mate waarin het PFS voorkomt bestaat onduidelijkheid,[2][8] deze symptomen komen echter zeer zelden voor. Vermoed wordt dat veel gevallen niet worden gemeld.[5] In 11 april 2012 bracht de FDA een verandering in de bijsluiter in de Verenigde Staten, daar werden onder andere het aantal van gemelde gevallen in de Verenigde Staten aangegeven..[15] In de wetenschap, en voor het belang van de patiënten, is verder onderzoek noodzakelijk.[1][2][5][8][16]

Oorzaken

De oorzaak van het zogenaamde PFS is niet bekend.[2][5][4][8] Finasteride remt de omzetting van testosteron naar DHT, maar verandert ook de testosteron, LH- en FSH waarden.[17] de biosynthese van DHT en diverse neurosteroïden zoals allopregnanolon [18] (ALLO) en Tetrahydrodeoxycorticosterone (THDOC), zijn afhankelijk van bekende isovormen van 5a-reductase. Ze zijn positieve allosterische modulatoren van de GABAA receptoren , die dezelfde actie van euforie en anxiolytica zoals benzodiazepines, bezitten. Omdat finasteride deze biosyntheses voorkomt, zoals aangetoond,[14][18][14][19] kan dit een van de oorzaken zijn voor de emotionele en seksuele bijwerkingen [12][14] Maar dit verklaart echter de andere symptomen, zoals spieratrofie, verlies van lichaamsbeharing en de voortdurende bijwerkingen zelfs lang na stoppen van medicatie, niet volledig. Aangezien veel patiënten een normaal tot hoog testosterongehalte en normale tot lage oestrogeenspiegel hebben en toch aan het klinische beeld van hypogonadisme voldoen geeft dit indicatie dat er een soort van androgene weerstand aanwezig is.

Gevolgen op lange termijn

Er is bewijs dat het gebruik van finasteride kan leiden tot een aggresieve, moeilijk te behandelen vorm van prostaatkanker kan veroorzaken bij mannen. kann.[20][21]

Therapie
